Professional Disclosure Statement

Introduction: Counseling can be conducted in a variety of ways, depending on the beliefs of the individual counselor. As my client, you have the right to know my qualifications, methods, and agreed mutual expectations of our professional relationship. Please feel free to discuss any questions or concerns you may have about our agreement.

Philosophy and Approach: Individuals come into counseling because they desire a change in their life. They may want to change their personal or family situation, solve a problem or work on a specific issue, change unhealthy habits or situations, or find acceptable balance and happiness.

During our first sessions, we will work together to assess your present needs and concerns and decide together and determine the need to continue with additional sessions. We will decide the appropriate time and methods of termination, which may include referrals to other professionals.

Throughout our time together, my goal will be to professionally facilitate to the best of my abilities, resolution of your concerns and needs. Ultimately, you are the one to decide if you are benefiting from our time together.

Formal Education and Training: I am receiving my Masters of Science at Oregon State University in Clinical Mental Health Counseling Program. I also hold a Bachelor’s of Arts Degree in Social and Behavioral Studies from George Fox University.  I am currently working on substance abuse and addiction credentials and I am a Qualified Mental Health Professional with a variance. I am experienced in the areas of anxiety, depression, PTSD, parental alienation, and substance abuse/additions.

As a client, you have the following rights:

  • To expect that a licensee has met the minimal qualifications of training and experience required by State Law;
  • To examine public records maintained by the Board and to have the Board confirm credentials of a licensee;
  • To obtain a copy of the Code of Ethics;
  • To report complaints to the Board;
  • To be informed of the cost of professional services before receiving the services;
  • To be assured of privacy an confidentiality while receiving services as defined by rule and law, including the following exceptions: 1) Reporting suspected child abuse; 2) Reporting imminent danger to client or others; 3) Reporting information required in court proceedings or by client’s insurance company; 4) Providing information concerning licensee case consultation or supervision; and 5) Defending claims brought by client against licensee;
  • To be free from discrimination because of age, color, culture, disability, ethnicity, national origin, gender, race, religion, sexual orientation, marital status, or socioeconomic status.

Fees: Fees for counseling services are determined by Now and Zen Parenting.
